Join our healthcare team as a part-time Home Health Nurse Practitioner, providing compassionate care directly in patients’ homes.
Conduct comprehensive assessments, develop personalized care plans, and perform treatments like IV infusions and wound care. Administer clinical procedures such as catheterization and phlebotomy, and educate patients on disease management and lifestyle modifications. Utilize EHR systems for documentation and coordinate with multidisciplinary teams for seamless care transitions. Require current NP license, experience in critical care preferred, and proficiency with EMR/EHR platforms. Strong assessment skills, compassionate communication, and the ability to work independently are essential.
