We are seeking a compassionate Speech Language Pathologist (SLP) for a home health/ home care position. This contingent role is ideal for a quick learner who enjoys teamwork and providing exceptional care.

**Compensation:**
– Visit rates with mileage reimbursement: $63.00 – $100.00 per visit
– Non-visit discharge: $5.00 – $10.00
– Non-visit transfer: $10.00
– Orientation and in-service hours: $30.00/hour
– Mileage reimbursement: $0.50 per mile after 30 miles/day
– Visit rates without mileage: $68.00 – $105.00 per visit

**Responsibilities:**
– Provide speech pathology or audiology services per physician’s plan of treatment
– Assess patient functioning and develop or revise care plans
– Document patient progress and communicate updates to the care team
– Educate patients, families, and staff on speech therapy and disease prevention
– Recommend hearing tests and check hearing aids as needed

**Requirements:**
– Active Michigan Speech Therapy license
– Reliable transportation, valid driver’s license, and auto insurance
– Home health experience preferred

**Travel:**
– Home visits and meetings as required, with reliable transportation essential.
