Home Health Speech Therapist

Job Description

This position is limited to Speech Therapists conducting home visits in Tucson, AZ.
In accordance with policies and regulations, provides age-appropriate, independent speech evaluation and treatment in patients’ homes.
Teaches patients, families, and caregivers preventative and rehabilitative procedures.
Develops and manages individualized care plans, coordinates treatment, and participates in interdisciplinary referrals and team conferences.
Regularly reassesses patient progress, adjusts treatment goals, and manages discharge planning.
Maintains accurate documentation and participates in clinical research and departmental programs.
Requires a minimum of 2 years of home health speech therapy experience, a valid license, driver’s license, and basic life support certification.
Willingness to travel up to 75%, with flexible hours (20-30/week), Monday to Friday, with weekends as needed.
Pay ranges from $80.00 to $90.00 per hour.
