We are a leading hospice organization committed to providing 24/7 in-home hospice care that supports patients physically, emotionally, and spiritually. Join our team as an Advanced Nurse Practitioner to oversee care plans, collaborate with physicians, and ensure compliance with regulations. Responsibilities include assessing patient needs, managing medications, completing face-to-face visits, and acting as the primary on-call clinician. Travel may be required.

Requirements:

  • Master’s Degree in Nursing and active license in the state
  • Current DEA license and at least 1 year of hospice or home care experience
  • Knowledge of hospice/palliative care principles, geriatrics, and regulatory standards
  • Preferred certification in hospice and palliative care
  • Ability to collaborate within an interdisciplinary team and communicate effectively with physicians
