Hospice Nurse Practitioner

Job Description

We are seeking a compassionate Hospice Nurse Practitioner to provide holistic end-of-life care in Greenfield, WI.

Conduct patient assessments and develop personalized care plans.

Administer medications and treatments as prescribed.

Collaborate with interdisciplinary teams to ensure optimal outcomes.

Educate patients and families about hospice services.

Monitor progress and adjust care plans accordingly.

Maintain accurate documentation and participate in discharge planning.

Advocate for patient needs within the healthcare system.

Qualifications include: a Master’s in Nursing focused on hospice or palliative care, a current WI Nurse Practitioner license, and relevant experience. Must possess strong clinical skills, excellent communication, and a compassionate approach. Ability to travel extensively is required, with mileage reimbursement provided.
