Seeking a Lead Physician to provide primary care and clinical leadership for a team serving vulnerable, complex patients in a comprehensive, community-focused setting.


  • Deliver excellent primary care and coordinate interdisciplinary teams to meet quality and utilization goals.
  • Mentor and coach providers, guide clinical practices, and ensure patient-centered, evidence-based care.
  • Lead clinical operations, develop protocols, and foster partnerships with community organizations and hospitals.
  • Manage acute and chronic conditions, prevent hospital readmissions, and address social determinants of health.
  • Participate in leadership activities, community engagement, and quality improvement initiatives.

    • Qualifications include an active medical license, 5+ years of clinical and leadership experience, strong communication skills, and a passion for population health.
