Job Description

We are hiring licensed clinical social workers (LCSW) for geriatric behavioral health in long-term care settings, supporting approximately 600 facilities across multiple states. This flexible role offers part-time or full-time hours in Fond Du Lac, WI, with daytime schedules and autonomy within facilities, backed by clinical support and an interdisciplinary team.

  • Provide direct psycho-social services, including evaluations, diagnoses, and therapy (individual, group, family) for nursing home residents.
  • Manage 10-15 patients daily, with scheduling flexibility to promote work-life balance.
  • Collaborate with a team to ensure quality care and proper documentation, including Medicare compliance.
  • Qualifications include an active LCSW license, 1-2 years’ experience, excellent communication, and tech proficiency.
  • Preferred experience in healthcare or nursing homes and familiarity with Medicare guidelines.
