Job Description

We are seeking a Part-Time LCSW Geriatric Psych Social Worker to provide psychosocial services in long-term care facilities in Manitowoc, WI.

Responsibilities include conducting evaluations, diagnosis, and treatment through individual, group, and family therapies for nursing home residents.

Collaborate with an interdisciplinary team to ensure quality care and continuity, documenting clinical contacts per Medicare guidelines.

Manage a flexible schedule, seeing 10-15 patients daily, with support from a dedicated practice manager.

Utilize electronic medical records and communicate effectively with clients and healthcare staff.

Qualifications include an LCSW license, a master’s degree, 1-2 years of experience, and proficiency with technology. Experience in healthcare settings and Medicare documentation is a plus.
