Licensed Clinical Social Worker (LCSW)

Job Description

We are seeking licensed Clinical Social Workers (LCSW) with a focus or interest in geriatric behavioral health for part-time or full-time roles in Fond Du Lac, WI.

Provide direct psycho-social services, including evaluations, diagnoses, and treatment for nursing home residents.

Conduct individual, group, and family therapies, seeing 10-15 patients daily with flexible scheduling for work-life balance.

Collaborate with interdisciplinary teams to ensure quality care and continuity, adhering to Medicare guidelines and proper documentation.

Enjoy autonomy within facilities, supported by a dedicated practice manager and modern electronic charting.

Qualifications include an active LCSW license, 1-2 years of experience, and strong communication skills. Preferred experience in healthcare settings and Medicare standards.
