Job Description

The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, family, and couples counseling, as part of an integrated primary care team.

They conduct assessments, diagnoses, and develop treatment and self-management plans, collaborating with the primary care team and external partners.

Responsibilities include crisis intervention, patient education, documentation in electronic health records, and participating in clinical meetings and quality improvement initiatives.

The role requires cultural competence, independent judgment, and adherence to organizational policies. It involves working in an office setting with some regional travel, primarily during daytime hours.
