Job Description
The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy—including individual, group, and family counseling—within an integrated primary care team.
They conduct assessments, diagnosis, develop treatment and self-management plans, and offer crisis intervention.
Working closely with clinic staff and external partners, they support patients’ behavioral and emotional health, aiming to improve overall well-being.
The role involves documenting in the electronic health record, collaborating with team members, participating in training, and contributing to quality improvement initiatives.
