Job Description
The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, and family counseling, as part of an integrated primary care team.
Conducts assessments, diagnoses, and develops treatment and self-management plans for adult and youth patients.
Offers crisis intervention, behavioral health education, and referrals to external providers as needed.
Collaborates with primary care providers and community partners to improve patient outcomes.
Maintains documentation in the electronic health record and participates in team meetings, trainings, and quality improvement initiatives.
Works in a culturally competent manner within a clinic setting, with some regional travel required.
