Job Description

We are hiring a part-time LICSW Social Worker specializing in Geriatric Psych for a growing long-term care provider supporting facilities across multiple states, currently in Minneapolis, MN.

  • Offer daytime hours with flexible scheduling, autonomy within facilities, clinical support, and a dedicated practice manager.
  • Provide direct psycho-social services, including evaluations, diagnosis, and therapy (individual, group, family) to nursing home residents, seeing 10-15 patients daily with flexibility.
  • Collaborate with interdisciplinary teams to ensure quality care and documentation following Medicare guidelines.
  • Qualifications include LICSW license, Master’s degree, 1-2 years experience, and proficiency with technology; experience with Medicare and healthcare settings is a plus.
