Job Description
We are hiring a Part-Time LICSW Social Worker specializing in Geriatric Psych for a growing provider supporting long-term care facilities in the Midwest, with an opening in Minneapolis, MN.
Provide direct psycho-social services, including evaluations, diagnosis, and treatment (individual, group, family) for nursing home residents. Manage 10-15 patient contacts per day, collaborating with interdisciplinary teams to ensure quality care and continuity. Maintain knowledge of Medicare guidelines and proper documentation of clinical contacts. Enjoy a flexible daytime schedule, autonomy within facilities, and support from a dedicated practice manager. Qualifications include a Master’s degree, LICSW license, 1+ years of experience (preferably in healthcare/nursing homes), and proficiency with technology.
