Job Description
We are seeking a Part-Time LICSW Social Worker specialized in Geriatric Psych for a growing provider supporting long-term care facilities in the Midwest, currently in Minneapolis, MN.
Provide direct psycho-social services, including evaluations, diagnosis, and treatment (individual, group, family) for nursing home residents.
Manage 10-15 patients daily with flexibility to adjust for work-life balance.
Collaborate with interdisciplinary teams to ensure quality care and continuity.
Maintain knowledge of Medicare guidelines and accurate clinical documentation.
Work daytime hours Monday-Friday with some flexibility; autonomy within facilities with clinical support.
Qualifications include LICSW, 1+ years’ experience, proficiency with technology, and relocation to Minneapolis, MN.
