The LCSW-Social Worker for Case Management provides comprehensive psychosocial support to patients with complex needs, collaborating with the care team and community resources to ensure safe discharge planning.
Offers a $3,000 sign-on bonus and a full-time position with pay ranging from $40.00 to $48.10 per hour.
Requires a Master’s Degree in Social Work, LCSW license, and current BLS certification.
Preferably has one year of supervised clinical experience in a medical setting.
Responsibilities include assessment, crisis intervention, referral, and care management to help patients and families cope with emotional and biopsychosocial issues.
