Job Description

The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with a multidisciplinary team to facilitate safe discharges and coordinate care.

Interventions include comprehensive assessment, referral, crisis intervention, and care management to help patients and families understand and cope with emotional, personal, and biopsychosocial issues impacting hospitalization.

Requirements include a Master’s Degree in Social Work, LCSW license, and current BLS certification. Preferred candidates have at least one year of supervised clinical experience in a medical setting.

The role offers a full-time schedule with a $3,000 sign-on bonus, hourly pay between $40.00 – $48.10, and benefits such as health, dental, vision insurance, paid time off, life insurance, and flexible spending account.

Work is conducted in person.
