Job Description

Seeking a full-time licensed clinician to provide mental health support at Elmwood Community Residence for adolescents aged 12-18 with Serious Emotional Disturbance (SED).

Responsibilities include clinical care coordination, facilitating treatment team meetings, collaborating with community providers, conducting screenings and intake, developing individual service plans, documenting progress, discharge planning, and leading groups. The role involves community work at family homes and partnering with peer advocates.

Candidates must be licensed (LMSW, LCSW, or LMHC), have a valid NYS driver’s license, pass background checks, and be on the DSS registry. Hours are Monday-Friday, with flexible scheduling including day and evening shifts and on-call duties.
