The Licensed Mental Health Professional will be a key member of an innovative Safe Options Support (SOS) team, providing comprehensive community care to street homeless or subway-dwelling individuals.
The multi-disciplinary team includes a leader, clinicians, care managers, and peers, utilizing the evidence-based Critical Time Intervention model.
Responsibilities include street outreach, coordinating needs before and after housing, improving daily living skills, offering supportive counseling, and advocating against discrimination and healthcare inequities.
The role emphasizes member choice, harm reduction, non-coercion, flexibility, and person-centered care.
Post-housing, the team supports participants for stability and independence over several months. The position involves fieldwork, on-call duties, and flexible hours.
