Licensed Social Worker

Job Description

We are seeking a dedicated Social Worker (CSW / LCSW) for a full-time role in Montgomery, PA. Working hours are 8AM – 4:30PM, in person.

Job Summary:
Assist in planning, developing, organizing, implementing, and evaluating social service programs within a facility. Support residents and families through care coordination, discharge planning, and connecting them to community resources to ensure positive outcomes.

Responsibilities include:

• Participating in community planning related to resident needs and facility services
• Assisting with discharge planning, care plans, and assessments
• Engaging residents and families in program planning
• Arranging transportation as needed
• Referring residents and families to community and social service agencies

Requirements:

• Bachelor’s degree in Social Work or related Human Services field
• At least 2 years of experience in a nursing facility
• Active CSW or LCSW license
• Strong communication, interpersonal, and conflict-resolution skills
