Job Description

We are seeking a full-time Social Worker (CSW/LCSW) to support social service programs in a healthcare facility located in Montgomery, PA. The role involves

  • Planning, developing, and evaluating social services and community programs
  • Supporting residents and families through care coordination, discharge planning, and connecting to community resources
  • Participating in resident assessments and care plan development
  • Arranging transportation and referring to relevant agencies

    • Requirements include a Bachelor’s degree in Social Work or a related field, at least 2 years of experience in a nursing facility, and an active CSW or LCSW license. Strong communication and conflict-resolution skills are essential. The position offers competitive pay, benefits such as 401(k), health, dental, vision, life insurance, and paid time off. The work schedule is 8AM-4:30PM, and relocation to Montgomery, PA, is required before starting.
