Job Description
This part-time social worker position requires a Bachelor’s degree in an accredited Social Work program and at least one year of experience in a medical setting. A Master’s degree and a valid Social Work license are preferred.
Responsibilities include providing social services aligned with the plan of care, supporting patients and families emotionally and socially, and preparing them for living situation changes. Assess social and emotional needs, evaluate caregiver capacity, and coordinate community resources. Maintain accurate documentation, prepare progress notes, and report patient updates regularly. Assist in staff training, participate in quality reviews, and develop care plans.
