Job Description
The Mental Health Therapist provides assessment, treatment, and collaboration with clients, families, and community agencies as part of a medically integrated care team.
They are responsible for conducting mental health assessments, supporting client engagement, developing care plans, and maintaining thorough documentation.
Additional duties include participating in supervision, treatment team meetings, case conferences, and consulting with colleagues and other providers.
The role involves delivering evidence-based clinical treatments, engaging with systems to ensure coordinated care, and contributing to team activities.
