Job Description
The Mental Health Therapist provides assessment, treatment, and collaboration with clients, families, and community agencies as part of a medically integrated care team.
Deliver a minimum number of client visits weekly and conduct mental health assessments. Support clients in overcoming barriers to engagement by coordinating with care team members across various systems. Develop and implement individualized care plans, maintaining clear documentation. Participate in supervision, team meetings, and case conferences, and collaborate with colleagues and other providers. Provide evidence-based clinical treatment and engage in ongoing training and consultation.
