Mental Health Therapist

Job Description

Seeking a dedicated *Geriatric Therapist* to provide individual, group, and family therapy to adults in a nursing home setting within PHP/IOP programs.

Develop, implement, and document personalized treatment plans in collaboration with an interdisciplinary team.

Ensure compliance with state regulations and maintain accurate documentation in the EHR system.

Engage residents in therapeutic activities to enhance emotional well-being and quality of life.

Participate in clinical reviews and team meetings for coordinated care.

Qualifications: Master’s in Social Work, Counseling, or related field; active Texas licensure (LCSW, LMSW, LPC, LMFT, etc.); preferred experience with geriatric populations or PHP/IOP settings; strong communication skills; familiarity with electronic charting.
