Job Description

Seeking a Neurology PA-C or Nurse Practitioner for a full-time field-based role in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs).

  • Perform comprehensive neurological assessments, diagnose and manage conditions like dementia, stroke, and Parkinson’s, and develop individualized care plans.
  • Order and interpret diagnostic tests, prescribe medications, and support patient and family education.
  • Collaborate with facility staff, participate in interdisciplinary meetings, and serve as a neurological resource.
  • Ensure accurate documentation, adhere to regulatory standards, and contribute to protocol development.
  • Require licensure, board certification, and 1-2 years of relevant experience; prior long-term care experience preferred.
