Nurse Practitioner

Job Description

The Nurse Practitioner (NP) role is part-time and focuses on providing primary and chronic care to elderly patients within the PACE program.

Deliver primary, acute, and chronic care in clinic, home, and facility settings.

Collaborate with the interdisciplinary team and under the Medical Director’s guidance.

Conduct assessments, develop care plans, manage medications, and coordinate care transitions.

Participate in team meetings and educate patients and families on health topics.

Accurately document in the EHR and support quality initiatives.

Requirements: Master’s degree in Nursing, active NP license, 2+ years in geriatrics, experience in hospice preferred, and passion for senior care.

Location: The role involves travel within multiple counties, treating participants both in the center and at home.

Benefits: $75,000/year with flexible schedule.
