Job Description

We are seeking a compassionate Nurse Practitioner to join our PACE team, providing primary and chronic care for older adults in clinic, home, and facility settings.

  • Deliver comprehensive assessments, develop individualized care plans, manage medications, and interpret diagnostics.
  • Collaborate with the interdisciplinary team and participate in care coordination and education for participants and families.
  • Document care accurately and support quality improvement efforts.

    • Requirements include a Master’s or Doctorate in Nursing, active NP license, and at least 2 years of geriatrics experience. Certification in Adult, Family, or Gerontology NP is preferred. The role involves working across multiple counties, including in-home and center-based care.

    This part-time position offers a $75,000 annual salary with a flexible schedule, in a mission-driven environment focused on helping seniors maintain independence and well-being. Candidates should be passionate about serving the elderly population and comfortable working both in clinics and in participants’ homes.
