Job Description

We are a rapidly expanding provider of innovative medical care for long-term and post-acute patients nationwide, utilizing advanced technology to proactively prevent hospitalizations.

We seek a full-time on-site Nurse Practitioner or Physician Assistant in Corsicana, TX, to deliver primary care in skilled nursing facilities. No weekends, nights, or holiday shifts required.

Responsibilities include conducting assessments, developing care plans, managing chronic and acute conditions, ordering diagnostics, prescribing medications, and educating residents and families. Collaboration with the healthcare team and participation in quality initiatives are essential.

Requirements include current Texas licensure, board certification, geriatric care experience preferred, strong communication skills, and proficiency with EMR systems. Occasional regional travel may be necessary.
