Job Description

We are a growing provider of innovative medical care for long-term and post-acute patients across the U.S., utilizing advanced technology to prevent hospitalizations.

We seek a full-time on-site Nurse Practitioner or Physician Assistant for Skilled Nursing Facilities in Corsicana, TX, with no weekends, nights, or holidays required.

Responsibilities include:

• Conducting comprehensive resident health assessments
• Developing personalized care plans
• Managing chronic and acute conditions
• Ordering and interpreting diagnostic tests
• Prescribing medications within scope
• Providing education to residents and families
• Collaborating with healthcare team members
• Participating in quality improvement efforts
• Maintaining accurate documentation

Requirements:

• Current Texas Nurse Practitioner license and board certification
• Geriatric care experience preferred
• Strong assessment and communication skills
• Ability to work independently and collaboratively
• Proficiency with EMR systems
• Occasional regional travel
