We are a rapidly expanding provider of innovative medical care for long-term and post-acute patients across the U.S., utilizing advanced technology to prevent hospitalizations and ER visits.

We seek a full-time on-site Nurse Practitioner or Physician Assistant to deliver medical care in Skilled Nursing Facilities in Corsicana, TX. This role offers a Monday–Friday schedule with no nights, weekends, or holidays.

Responsibilities include conducting health assessments, developing care plans, managing chronic and acute conditions, ordering diagnostics, prescribing treatments, and providing patient and family education. Collaboration with interdisciplinary teams and participation in quality initiatives are essential.

Requirements: Current Texas licensure and board certification as a Nurse Practitioner, geriatric care experience preferred, strong assessment and communication skills, proficiency with EMRs, and ability to travel regionally.
