Job Description

We are seeking a compassionate Nurse Practitioner to join our PACE program, providing primary and chronic care for older adults in clinic, home, and facility settings.

  • Deliver comprehensive assessments, develop care plans, manage medications, and coordinate care transitions.
  • Collaborate with an interdisciplinary team and participate in team meetings to ensure person-centered care.
  • Educate participants and families on health conditions, prevention, and wellness.
  • Accurately document in the EHR and support quality improvement initiatives.

    • Qualifications include a Master’s or Doctorate in Nursing, active NP license, with experience in geriatrics or primary care.

    Enjoy a full-time role with a $150k salary, benefits, flexible schedule, and meaningful work supporting seniors to remain independent. Must be able to commute daily to Newaygo, MI. Passion for serving seniors in a value-based care model is essential.
