Job Description

This part-time Nurse Practitioner role in the PACE program involves providing primary, acute, and chronic care to elderly participants in clinic, home, and facility settings.

  • Collaborate with an interdisciplinary team under medical direction to develop and implement personalized care plans.
  • Conduct assessments, manage medications, order diagnostics, and coordinate care transitions.
  • Educate participants and families on health, prevention, and wellness strategies.
  • Document care accurately in the electronic health record and support quality improvement efforts.

    • Ideal candidates have a Master’s or Doctorate in Nursing, an active NP license, and at least 2 years of geriatrics experience (preferably including hospice). They should have strong clinical judgment, communication, and teamwork skills, with a passion for serving seniors in a value-based care setting.

    The position offers a $75,000 annual salary, flexible schedule, and requires treating participants both in the clinic and in their homes across multiple counties in Michigan. Candidates must be willing and able to meet these requirements and have a passion for elder care.
