Job Description

This Nurse Practitioner position offers a starting salary of $100,000 annually and involves providing primary care across multiple sites in South Carolina, including Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.

As a vital team member, the NP will develop and oversee a community-based primary care program focused on family-centered services. Responsibilities include designing an integrated care model emphasizing prevention, health maintenance, education, and evidence-based chronic disease management, in collaboration with a multidisciplinary team.

The ideal candidate is committed to patient-centered care, with strong communication and interpersonal skills to build relationships with diverse populations. They should be knowledgeable in current primary care practices, preventive medicine, and chronic disease management.
