• Responsibilities: Conduct patient assessments, develop care plans, collaborate with the team, educate patients and families, and monitor progress.
• Qualifications: Valid Pennsylvania Nurse Practitioner license, with preferred experience in skilled nursing facilities, and ability to work collaboratively.
