Nurse Practitioner

Job Description

Seeking a compassionate Nurse Practitioner to join a PACE program dedicated to providing comprehensive care for seniors.

Deliver primary, acute, and chronic care in clinic, home, and facility settings.

Collaborate with an interdisciplinary team and under physician guidance.

Conduct assessments, develop care plans, manage medications, and interpret diagnostics.

Participate in team meetings to ensure person-centered, integrated care.

Educate participants and families on health and wellness.

Accurately document care and support quality improvement efforts.

Qualifications: Master’s or Doctorate in Nursing, active NP license, with experience in geriatrics or chronic disease management, and strong clinical judgment. Certification in Adult, Family, or Gerontology NP preferred.

Note: Must be able to commute to Newaygo, MI, and work at the center location. Passion for senior care in a value-based model is essential.

Experience in primary care and chronic disease management for at least 1 year required.
