Join a healthcare team providing patient care in post-acute facilities, where Nurse Practitioners and Physician Assistants collaborate to improve outcomes.

The Advanced Practice Provider role involves managing and coordinating patient care within an assigned population, with comprehensive training, shadowing, and oversight by a Medical Director to ensure support and preparedness.

This position requires:
– Coordinating and delivering quality patient care
– Effective communication with staff and physicians
– Conducting patient rounds, making care decisions, writing orders, and scheduling tests
– Timely documentation in electronic medical records
– Maintaining accurate service records for billing and compliance

Skills needed include excellent communication, empathy, teamwork, organization, time management, and problem-solving. Preferred experience in geriatrics, internal medicine, ICU, emergency medicine, or hospitalist settings is advantageous. Must hold a Nurse Practitioner license and DEA certification.
