Nurse Practitioner CRNP

Job Description

The Nurse Practitioner (CRNP) provides primary care and case management for participants in a community-based program serving elderly residents in western Pennsylvania.

Responsibilities include managing health, preventing disease, and coordinating care with an interdisciplinary team to support participants’ independence at home.

Qualifications:

Graduate of an accredited nursing school, licensed RN in Pennsylvania, with NP certification

Collaborating physician agreement and at least one year of experience with the elderly

Knowledge of community health, care management, and working with cognitively impaired seniors

Strong communication skills, flexibility, and computer proficiency
