Job Description
The Nurse Practitioner (CRNP) provides primary care and case management for participants in a community-based program serving elderly residents in western Pennsylvania.
Manage health care, focusing on disease prevention, wellness, and chronic/acute illness management in collaboration with primary care providers and the medical team.Work within an interdisciplinary team to develop personalized Plans of Care aligned with participant goals.Require licensure as an RN with advanced practice certification, experience with the elderly, and knowledge of community health and care management.Offer holistic, person-centered care while maintaining flexibility and strong communication skills.Benefits include competitive wages, health and insurance plans, retirement options, tuition reimbursement, and wellness resources.
