Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, diagnostics, and develop treatment plans.

Manage acute and chronic conditions, prescribe medications, and educate patients and families.

Collaborate with care teams to ensure seamless, patient-centered care.

Offer autonomy with flexible scheduling, minimal on-call, and a supportive environment.

Qualifications include Minnesota licensure, board certification, and preferably 1+ year of experience in a geriatric or post-acute setting.
