We are seeking an experienced Nurse Practitioner for a mobile primary care role in skilled nursing, long-term care, and assisted living facilities.
Provide comprehensive assessments, diagnose, and manage acute and chronic conditions. Order diagnostics, develop treatment plans, and prescribe medications within guidelines. Collaborate with care teams to ensure seamless patient care and education. Maintain accurate documentation and support quality improvement efforts. Qualifications: MN Nurse Practitioner license, board-certified. 1+ year experience preferred, especially in geriatric or post-acute settings.
