Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in skilled nursing, long-term care, and assisted living facilities.

  • Perform patient assessments, diagnostics, and develop treatment plans for acute and chronic conditions.
  • Manage medications, provide education, and collaborate with care teams for seamless patient care.
  • Enjoy autonomy in clinical practice, flexible scheduling, and a supportive environment.
  • Qualifications include MN NP license, board certification, and preferred geriatric experience.
