Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities. This full-time role offers autonomy, flexible scheduling, and a supportive team environment.

Responsibilities include conducting patient assessments, managing acute and chronic conditions, ordering diagnostics, prescribing medications, and collaborating with care teams to ensure seamless, patient-centered care. Strong communication and geriatric experience preferred.
