Requirements:

Certified Nurse Practitioner license

preferred 2 years’ experience in Skilled Nursing facilities

excellent communication, organization, and time management skills

ability to travel within 12-20 miles

Responsibilities include collaborating with patients and teams, developing treatment plans, conducting assessments, diagnoses, treatments, and maintaining accurate records.
