Job Description

We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full-time role offers flexible scheduling, including part-time and per diem options, with a typical Monday–Friday schedule and some weekend/holiday coverage.

Key responsibilities include conducting assessments, developing treatment plans, managing medical conditions, prescribing medications, and collaborating with interdisciplinary teams to ensure high-quality resident care. The NP will also document in the EHR, participate in care team meetings, and support nursing staff.

Qualifications required are an active NY NP license, certification as an Adult-Gerontology or Family Nurse Practitioner, DEA registration, and BLS certification. Preferred experience includes geriatrics or long-term care.
