Job Description

We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full-time position offers flexible scheduling, with options for part-time or per diem. The NP will collaborate with the healthcare team to conduct assessments, develop treatment plans, manage medical conditions, and coordinate care. Key responsibilities include documentation, ordering tests, prescribing medications, and participating in quality improvement activities.

Qualifications include an active NY NP license, certification as an Adult-Gerontology or Family Nurse Practitioner, DEA registration, and at least 1-2 years of geriatrics or long-term care experience. Strong clinical, communication, and organizational skills are essential.
