Job Description
We are seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered primary care to PACE participants in clinics and homes.
Conduct comprehensive assessments and manage acute and ongoing conditions. Develop and revise individualized care plans aligned with patient goals. Provide preventive services, immunizations, screenings, and medication management. Order diagnostics, facilitate referrals, and educate participants on therapies and diets. Participate in on-call rotations, home visits, and care coordination within care settings. Collaborate with the interdisciplinary team and manage care in contracted facilities. Requirements: Valid NP license, board certification, DEA, BLS, at least 2 years’ experience, and willingness to travel and relocate to Kokomo, IN.
