Job Description

Join a dedicated healthcare team providing exceptional post-acute care in community facilities as an Advanced Practice Provider, including Nurse Practitioners and Physician Assistants.

Coordinate and deliver patient care, conduct rounds, and collaborate with staff and physicians.

Maintain accurate documentation, write orders, and facilitate tests and procedures.

Ensure high-quality outcomes with compassionate communication and teamwork.

Participate in comprehensive training, shadowing, and oversight to support professional growth.

Required: Nurse Practitioner license, DEA certification, excellent communication, empathy, organization, and prior relevant experience preferred.
