Job Description

Join a dedicated healthcare team providing exceptional post-acute patient care within the community. As an Advanced Practice Provider, you’ll manage and coordinate patient care, conduct rounds, communicate with staff, and ensure accurate documentation. The role offers comprehensive training, shadowing, and oversight by a Medical Director to support your success.
