Job Description
We are seeking a compassionate Nurse Practitioner to join our mobile wound care team, delivering patient-centered services across various care settings.
Provide on-site assessments, debridements, and treatments for patients in skilled nursing facilities, assisted living, and private homes. Collaborate with healthcare teams to develop and implement care plans, monitor progress, and adjust treatments as needed. Educate patients and families on wound care and prevention strategies. Accurately document all visits in EMR systems and ensure compliance with regulations. Maintain current licensure in Missouri and Illinois, with preferred wound care experience or training. Must be willing to travel for training and have reliable transportation.
